CHANGES TO THE ADVICE LINE

Our Doctors and Nurse Practitioners provide an emergency telephone advice line, so if you are looking for emergency advice only, please call the surgery between 2pm-4.30pm and leave your details and a brief summary of the problem. A clinician will return your call within 24 working hours. Please provide a telephone number where we can reach you between 9am and 6pm.

PLEASE BE AWARE THE NEW CLOSING TIME OF THE ADVICE LINE IS 4.30PM NOT 5.30PM

How to register as a patient

First of all, you will need to check with the receptionists that you live in our catchment area. In order to register you will also need to provide evidence of your identity (e.g. passport or driving licence) and evidence of your address (e.g. paid utilities bill, council tax bill, tenancy agreement)

When you register you will also be asked to complete a New Patient Questionnaire. This can be given to you, at the time, by our Reception staff but, if you would prefer to print it off and complete your form in advance please do so.

It may take some time before your medical notes reach us from your previous practice. The information that you provide on your New Patient Questionnaire is essential for your new GP, and enables them to continue your health care whilst we await your notes.

Please note that unless you are a UK Resident you cannot be registered as a permanent patient at this Practice. If you are a non-permanent UK resident you may be able to register as a Temporary Patient if you have an urgent need to see a GP. You may be asked for additional evidence of status, such as a visa or work permit, in order to allow you permanent registration.
